Problem Note 40397: After saving a new questionnaire object, the questionnaire remains locked

In SAS® OpRisk Monitor, all questionnaire object locks should be released after saving a risk. However, when you perform the following steps, a lock remains on the questionnaire object:

For software releases that are not yet generally available, the Fixed
Release is the software release in which the problem is planned to be
fixed.
- Create a form-based assessment and continue to scoping.
- Add an assessor.
- Add a risk from the library.
- Edit the risk and make a change (for example, change the risk name or description).
- Click Save.
Because the questionnaire is still locked after the save, when you edit a risk on an assessment and then send the assessment for validation, the validator will get the following message:
Questionnaire questionnaire-ID is already locked by assessor-name
To work around this problem, delete the lock from the Administration ► Locked Objects screen.
