This problem occurs when a reference to a connection factory is used rather than a reference to the connection factory manager. An error on another thread can result in the connection factory shutting itself down, and the getConnection call fails with an error message containing the following:
Caused by: java.lang.IllegalStateException: "getConnection()" cannot be called on a factory after "shutdown()" or "destroy()" has been called.
To recover, restart the metadata server. Note that the dependent servers should be stopped in the correct order as shown below and restarted in the reverse order:
