Problem Note 16792: Duplicated BYLINE appears in PRINTER output when PROC PRINT is used
When PROC PRINT with a BY statement is used to route output with
multiple pages to one of the ODS PRINTER destinations (PCL/PDF/PS), the
first table on the second or subsequent pages may be missing it's
headers and have the BYLINE repeated.
The workaround is to recode using PROC REPORT.
NOTE: since BY groups automatically generate pagebreaks for REPORT,
STARTPAGE= may need to be set to NO/OFF in order to mimic PRINT's
pagination.
