If you attempt to use an external function in your MDX query, you may
receive the following error in your SAS OLAP Server log:

   20041109:10.07.59.21: 00001827: 21:sasadm: IOM RETURN -1=
   {compRef:6514320}->MDDataSet::Init() : text=<?xml version="1.0" ?
   ><Exceptions><Exception><SASMessage severity="Error">Access
   violation</SASMessage></Exception><Exception><SASMessage
   severity="Error">An OLAP server thread has
   failed</SASMessage></Exception></Exceptions>

Or, if you query the cube using SQL Passthru, you may receive errors
such as:

   ERROR: An OLAP server thread has failed
   ERROR: Access violation
   11 EXECUTE(DROP LIBRARY 'Hello') by olap;
   ERROR: A run-time execution error occurred in the MDX query
   ERROR: A run-time execution error occurred in the MDX query
   ERROR: Function library "Hello" is not loaded; therefore, it cannot
          be dropped
   ERROR: The immediate MDX execution failed
