EPTO Account Status Request Form
Complete the following information to receive an e-mailed EPTO account
usage report within 2 business days. The report will contain your current
EPTO status which includes the number of remaining EPTO units available, a
usage summary with the student's name, class taken, the class date, and
number of EPTO units used.
Please note that this report will be e-mailed only to the EPTO account
administrator we have on file.
