For good and valuable consideration, I hereby grant SAS Institute Inc., its licensees, subsidiaries, affiliates, successors, and assigns (collectively "SAS") the right to publish and release my name, past, present, and any future certification status under the SAS Institute Inc. Certification Program, and state/province and country of residence (collectively "Certification Information") in perpetuity throughout the world, in and through all media or means of communication now known or hereafter created.

I release SAS, its subsidiaries and affiliates and their employees, successors, and assigns from any claims of damages for libel, slander, invasion of rights of privacy or publicity, and any other claim based on the publication or release of any Certification Information as specified in this Certification Information Release. This Certification Information Release shall be binding upon me and my heirs, legal representatives, and assigns.

This form only needs to be completed once for your name to appear on the directory. All subsequent credentials earned will appear on the directory automatically; therefore, an additional form does not need to be completed.
